Athletics provide an opportunity for students to learn lifelong skills, like accountability, sportsmanship, integrity and self-determination. Our students become models for others – leading the way in the classroom, on the field and in the community.
Any high school student interested in participating in Phoenix athletic sports programs must have the following completed prior to the start of practice for each sport they intend to participate in. Please note, this does not apply to middle school student athletes.
- Sports Physical: Student Athletes must have a sports physical completed by their physician and have a Physical Form and Clearance Form completed and signed by the physician.
- Parents must fill out and sign the bottom portion of the Clearance Form in the Sports Physical paperwork.
- Return the completed Physical Form and Clearance Form to Milwaukee Excellence prior to the student’s first practice. Students will not be allowed to participate in practice without all of the proper paperwork on file with Milwaukee Excellence.
Please Note: Physicals completed before April 1st of the school year are valid for the current school year and the following school year. Physicals completed after April 1st of the school year are valid for the following two school years.
